One Life is dedicated to protecting your privacy. We do not disclose or sell our customers’ nonpublic personal information to anyone except as permitted or required by law. If a state’s law is more restrictive than Federal law, we follow that state’s privacy law when doing business in that state.

We collect personal information as part of our insurance business. Most of this comes from your application. We keep information about the products and services you purchase from us. We also collect personal information from outside sources, including consumer reporting agencies and health care providers. This information may include financial and medical information. Information obtained from insurance support groups may be retained and later disclosed to other such groups. Information obtained from a report prepared by an insurance support organization may be retained by that organization and disclosed to other persons.

We protect your information in several ways. When nonaffiliated companies work with us, we first require them to enter into a confidentiality obligation. Within One Life, your personal information is limited to those who require it to service your needs. In addition, we train our employees to protect information, and we have established physical and electronic safeguards. Should your relationship with One Life end, your information will remain protected in accordance with our privacy practices.

You have the right to access and request correction of your nonpublic information as recorded by One Life. Any such request must be specific as to what information is being requested and should be directed in writing to One Life’s administrative office:
